Mr. McGehee,

I just want to make a very brief comment on your post. I have never read
a more precise, a more accurate description, and a more valuabe diagnosis,
of the causes of operational failure. (Note, I did not qualify that to mean
CIA, FBI, etc...) But I did  mean Government, not private sector... I think
your "diagnosis"  holds true for all forms of Government, from City blunders
to the Federal circus.
